The first row is the prescription of the right eye (OD) and the second row is the left eye (OS). Within these rows are three values: Sphere (SPH), Cylinder (CYL), and axis. The SPH corrects for near sight, and CYL / Axis indicates astigmatism. You might have another field labeled as ADD which corrects for reading power used in a progressive or bifocal lens.
